I honestly have no clue. Although loudest-subtext said once that she thinks there will be a romantic/sexual cliffhanger that works in tandem with the plot cliffhanger, and after the way HLV ended, I agree.
When Sherlock says the “girl’s name” thing, it’s funny and frustrating as all hell. But that wasn’t just their way of baiting the fans. In that moment after he says “there’s something I should say,” everyone – John included – knew exactly what he meant. He wasn’t just setting up a joke. He was saying goodbye, and part of him thought well, why not just go for it…
And John knew it was coming.
"I meant to say always and I never have. Since it’s unlikely we’ll ever meet again, I’ll say it now."
John is waiting for it. Looking him in the eyes, no less, in such contrast with that moment at the end of TSoT where Sherlock’s feelings were written all over his face and John had to look away. But John can face it now.
And Sherlock rethinks it.
Leave John with a confession of love, with so many questions that would forever go unanswered? No way. We would never do that to John Watson. Let him go.
So he makes the joke, and the tension breaks. John laughs. Relieved, disappointed? Probably both.
But the point is, it was right there between them for a moment, unspoken. I love you.
Sherlock didn’t say it, not yet, but he put the thought out there, and that’s something.
So in season 4, they’ll have to up this. A confession, an almost-kiss, I don’t know – some sort of romantic and/or sexual cliffhanger a level above this one to keep us guessing as to where their relationship is headed in season 5.
